A Near-Fatal Injury

The 30-year-old sustained the injury during the third ODI against Australia at the Sydney Cricket Ground on October 25 while taking a catch running backward from point to dismiss Alex Carey. He immediately clutched his rib cage after completing the catch and was taken off the field.​

Scans revealed a lacerated spleen with internal bleeding—a potentially fatal condition that required immediate surgical intervention. According to reports, Iyer’s oxygen levels had dropped to 50, and for ten minutes he couldn’t stand properly. “There was a complete blackout around him and it took him some time to come back to normal,” a BCCI source told Indian Express.​

Medical Treatment and Recovery

The injury was promptly identified, and bleeding was immediately arrested following a minor procedure. BCCI doctor Rizwan Khan stayed with Iyer at the Sydney hospital throughout his treatment, coordinating with specialists Dr. Koroush Haghighi in Sydney and Dr. Dinshaw Pardiwala in India.​

On November 1, the BCCI announced Iyer’s discharge from the hospital, stating he was stable and recovering well. However, he remained in Sydney for follow-up consultations before being cleared to fly home.​

Understanding the Injury

Dr. Kaushal Pandey, a medical professional who explained the injury on social media, described the spleen as an “immune powerhouse” located in the upper left abdomen below the rib cage. Its position makes it vulnerable to blunt trauma during high-impact sports.​

“Imagine this—a fast-rising ball hits your left side and your abdomen’s organ silently bleeds. That’s exactly what happened to Shreyas Iyer’s spleen,” Dr. Kaushal explained. Such impacts can cause subcapsular hematomas, lacerations, or complete ruptures, with internal bleeding often being “slow and sneaky”.​

Modern medical practice favors preserving the spleen whenever possible because it’s crucial for immunity. “Losing it increases lifelong risk of severe infections,” Dr. Kaushal noted.​

Recovery Timeline

For professional athletes, recovery timelines vary by severity. Low-grade spleen injuries allow resumption of activity in about six weeks, while high-grade injuries can take three to four months. Sources indicate Iyer will be out of action for at least the next few months to achieve total recovery and match fitness.​

The injury puts his participation in the ODI series against South Africa at home—starting November 30 in Ranchi, Raipur, and Visakhapatnam—in serious jeopardy. “He will take more time to be fully match fit and the board and selection committee doesn’t want to rush after his injury. He is doubtful for the South Africa ODI series,” a BCCI source told Indian Express.​

Impact on Indian Cricket

Iyer’s absence is a significant blow to India’s middle order. In 2025, he scored 496 runs in 11 matches at an average of 49.60, including five fifties. During the Champions Trophy-winning campaign earlier this year, he was India’s top run-getter with 243 runs in five innings.​

Overall, Iyer has amassed 2,917 runs in 73 ODIs at an average of 47.81, including five centuries and 23 fifties. His ability to anchor innings and accelerate when needed makes him irreplaceable in India’s middle-order plans.​

While Iyer’s latest Instagram post suggests he’s on the mend, a full return to international cricket remains weeks or months away. The BCCI and selection committee are taking a cautious approach, unwilling to risk his long-term health by rushing him back prematurely.
